Community Health Worker
*Position Description:*
Under the supervision of the Executive Director, the Families First Community Health Worker (CHW) will provide support services, case planning and resource referral for individuals and families in emergent situations or in obtaining/maintaining their basic needs through assessment, advocacy, collaboration, education and support.
The CHW will utilize best practice strategies by implementing the Pathways Community HUB Mode to deliver services and will partner with the Great Rivers HUB to deliver care.
This position requires strong communication, time-management, organizational and conflict resolution skills with the ability to problem solve and react quickly to crisis situations.
The CM is an integral part of a team, both on site and within the community and is expected to participate in activities that will advance the mission of Families First and the quality of services and programming that benefit the recipients of our care and support.
*Position Responsibilities:* Provide high quality service to all clients, partner agencies, and community members.
Acts upon critical needs appropriately and immediately when presented.
Schedules and conducts initial needs assessments, in person or telephone, with individuals or families.
• Orientates clients to Families First programs.
Educate clients on the intake/application process, case planning, development, and management, on community resources and the referral process.
Provide supportive case management services for clients who are in emergent situations, such as, homeless, or at-risk of homelessness, or in need of basic life essentials by developing a client-centered case plan with goals and objectives.
Document all case management services in Care Coordination System database.
Establish, maintain, and act as a liaison between Families First and contract or collateral agencies to coordinate services.
Advocate on the client’s behalf with appropriate agencies, landlords, utility vendors, companies, or any client identified agency or individual to document need or type of assistance and negotiate the Complete administrative duties appropriately and according to funding guidelines and state and federal laws.
Participate in meetings and committees as required by the Executive Director or as needed in support of Families First clientele.
Maintain knowledge of community resources, especially those related emergent situations, low-income and at-risk/homeless families, and individuals.
Possess a high level of individual initiative and creativity.
Strong correspondence and editing skills.
Ability to work with people from wide varieties of circumstances in supportive, non-judgmental manner, being able to assist them in recognizing their needs and objectives and how to meet them.
Ability to work under pressure and be flexible.
Valid driver’s license, reliable transportation, and driver’s liability insurance *Education, Certifications, and Training* High School diploma or equivalent.
Complete Community Health Worker training provided by Great Rivers HUB and complete all an-going trainings to maintain certification.
Knowledge of community problems in fields of health, welfare, social, and economic problems, and how they affect families and individuals and experience related to low-income and “at risk” families and individuals.
Experience working with the homeless or low-income individuals and families.
Sensitivity to issues surrounding individuals and families experiencing homelessness, substance abuse, mental illness, or physical ailments.
Experience in crisis intervention and problem solving with ability to diffuse situation without heightening the conflict.
Computer skills with knowledge of Microsoft Office 365.

Individual to hold this position will be exposed to multifaceted work environment and will need knowledge and experience in using a multi-line telephone, calculator, multi4unction copy machine, computer, and other office equipment.
Sit for regularly scheduled periods of times to complete paperwork and perform work on the computer.
Apply manual dexterity and visual acuity to review detailed information and material.
Physical ability to move around the office area repeatedly throughout the day.
Individual will perform work in office and may be required to travel to meet with clients in other community settings.
