Schedule Weekly Hours: 40 This social work position will be an integral part of our Adult Partial Hospitalization Program (PHP) and Mental Health Day Treatment Program (MHDTX).
We are looking for someone who is self-motivated and passionate about working with patients in Behavioral Health.
What you will work: 1.
0 FTE = 80 Hours every 2 weeks Days, Monday - Friday Location is Onalaska, Annex What you will do: Receive and screen PHP referrals Schedule patients in Epic Provide care coordination for patients in PHP and MHDTX Facilitate multiple groups per week in PHP and provide group coverage when needed for PHP and MHDTX What you will get: Department with a strong teamwork model Gundersen's generous compensation and benefit package Growth opportunities and access to Gundersen's Career Development Center to help you navigate your career What you will need: Bachelors degree in Social Work or in a related field WI Social Worker license 1 year of previous experience in human services or health care Job Description: Responsible for responding to caller's requests to Behavioral Health for services including; screening for appropriate level of care and type of service, scheduling of appointments, assuring appropriate patient flow into and through department, information and referrals for adjunct support and services, follow-up calls, brief telephone counseling and brief crisis intervention making appropriate referrals to triage psychotherapist as needed.
Major Responsibilities: 1.
Assessment and Triage: Screens callers for appropriate type of service and level of care or assessment.
Works with other healthcare professionals to identify potential patients through screening, consultation, and referral.
Provides brief telephone and/or face to face counseling and crisis intervention making referrals to triage psychotherapist and/or psychiatrist as needed.
Schedules appointments with appropriate providers following screening call/contact.
Provides crisis intervention and counseling in collaboration with other healthcare professionals to provide emergency assessment for the patient and family.
Sorts all incoming Epic orders and assigns to appropriate provider.
Participates in triage staffing with team and manager to review calls and cases to assure quality.
Provides appropriate documentation using patient contacts to communicate clinical information to providers and when appropriate referral sources.
Demonstrates effective communication with consumers, triage team and referral sources with an emphasis on customer service.
Is knowledgeable of and utilizes resources to assure appropriate treatment of identified problems and to assist with patient and family referral.
As assigned, assists with the collection of patient volume and other relevant statistics.
Manages waiting lists for services, utilizing urgency criteria to assign new appointments.
Follow Standard Operating Procedure for all patient requested transfer requests, patient concerns and patient grievances.

Support Gundersen Behavioral Health Providers: Be available for high acuity patients’ appointments as identified by providers.
Obtain collateral information from various community agencies and programs at the request of providers.
Assist with referrals for therapy, community programming, and possible County involvement.
Collaborate with outside agencies (i.
e.
schools, local counties, therapy agencies) for wrap around care coordination.

Treatment Readiness Assessment: Works with patients/family members to determine concerns, strengths and weaknesses, patient motivation, barriers to treatment, and appropriate level of care.

Case Management: Partners with patients and families to monitor service delivery and ensure services plan goals are being met.
Provides supportive counseling as needed.
Provides referrals for adjunct services when appropriate.
Attends department meetings, developing and maintaining professional working relationships with providers.
Performs other job-related responsibilities as requested.
Education and Learning: REQUIRED Bachelor's degree in Social Work or a related human service field Work Experience: REQUIRED 1 year of previous experience in human services or health care.
DESIRED 1 year of experience with direct patient contact.
Interviewing/supportive counseling experience.
Crisis Intervention experience.
License and Certifications: REQUIRED Social Worker licensed in the state(s) of practice Age Specific Population Served: Adolescent (13 - 17 years) Adult (18 - 64 years) Geriatric (65 years & older) OSHA Category: Category III - No employees in this job title have a reasonably anticipated risk of occupational exposure to blood and/or other potentially infectious materials.
