50 FTE, 40 Hours a Pay Period Days, Hours between 8 am - 4 pm Weekends: No Weekends Holidays: No Holidays Telecommute Available: Partial Position Overview: The Master Social Worker (MSW) serves both the in-center hemodialysis department and the outpatient nephrology department.
In this role, the MSW provides case management and care coordination for patients with end stage renal disease who are receiving hemodialysis as well as patients seeking nephrology care for any stage of kidney disease or kidney function problem.
The MSW coordinates with the other members of the interdisciplinary hemodialysis/nephrology team; which includes the Medical Director, Nephrologist, Nurse Practitioner, nursing staff, dialysis technicians, dietitian, administrative support, and management.
The MSW also interacts with a variety of community organizations and services to support the needs of the patients served.
The MSW will periodically attend the Dialysis Quality Assessment and Performance Improvement team.
The MSW also participates in departmental and system-wide quality improvement activities.
Essential Duties & Responsibilities: Provides social work services consistent with patient goals of care; case management, assessment, care planning, discharge planning, financial aid coordination, end of life support, dialysis initiation adjustment support, kidney transplant referral and coordination, education and referral support for alternate modalities of renal replacement therapy.
Consults with physicians and co-workers as a member of the interdisciplinary team in the in-center hemodialysis department and outpatient nephrology department.
Participates in and supports team and patient in planning patient care.
Regularly participates in team meetings with patient/family-system to facilitate communication and implement treatment recommendations.
Serves as an advocate for the patient and patient's rights.
Is knowledgeable of and keeps current on available programs and resources to assist patient/family and staff in utilizing community resources.
Provides information and referrals for the patient to community resources.
Documents all sessions with patients/families completely and in accordance with all regulations and organizational policy in the patient’s clinical electronic medical record.
Maintains service excellence by exhibiting excellent customer relations with patients, families, visitors, schools, other organizations, physicians, co-workers, and community and referral services.
Responsible for personal and professional development to ensure current knowledge and performance in the profession/position.
MSW is responsible for maintaining appropriate licensure to practice as a social worker at the MSW level as licensed by the Minnesota Board of Social Work.
Complies with established departmental policies and procedures.
Performs the KDQOL assessments and reports monthly to QAPI.
Complies with all safety, environmental, and infection control standards.
Other duties as assigned.
Essential Skills and Experience: Master’s Degree in Social Work (MSW) Current valid license or ability to become licensed by the Minnesota Board of Social Work as a LGSW, LISW, LICSW Certification as an Advanced Care Planning Facilitator is preferred upon hire and/or requires to obtain within 6 months of hire Wisconsin Social Work license is preferred upon hire (CAPSW, CISW, or LCSW) or required to obtain within 6 months of hire Valid driver’s license Basic Computer Skills: Word, Excel, Outlook, and Patient Information Database Must pass Ishihara Test for Color Deficiency prior to employment Nonessential Skills and Experience: Previous experience in Nephrology/Hemodialysis Social Work is preferred but not required Experience working with patients at end of life in a palliative care or hospice environment or in chronic disease management is preferred but not required Internal Applicant Policy: It is the policy of Winona Health to work with employees in an equitable fashion regarding promotions, transfers, and position reclassifications.
