TITLE: On-Call Crisis Response Practitioner (overnight hours) PROGRAM: Community Based Services JOB SUMMARY: To handle on-call crisis intervention and stabilization proceedings for Houston, Wabasha, and Winona Counties.
JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS: Assume responsibility for being on-call evenings and weekends as days scheduled.
Respond to on-call crisis calls and assess the situation for further follow-up.
Provide support and utlize active listening skills.
Stay with individuals in a crisis bed setting or in their homes.
Develop plans with individuals to assist them in restoring his/her prior functioning level following a crisis.
Attend monthly On-Call Crisis Team meetings.
Must be available to work irregular hours and be flexible with scheduling.
Complete the On-Call Crisis Training.
Possess a vehicle, valid driver license, and valid auto insurance.
PHYSICAL REQUIREMENTS FOR POSITION: Must be able to walk, sit, stand, crouch, twist, bend, reach overhead, maneuver stairs and stretch in a manner conducive to the execution of daily activities.
Must be able to move around the office on a daily basis.
While performing the duties of this job, the employee must communicate with others and exchange information.
This is primarily a sedentary position (standing and/or sitting).
Employee regularly operates a computer and other office equipment on a daily basis.
Occasional bending and lifting of office materials up to 30 lbs.
NON-ESSENTIAL FUNCTIONS: Perform other duties as assigned by the Program Director or Crisis Team Coordinator EQUIPMENT USED: Telephone/cell phone, computer, office equipment, and conference calling, vehicle.
JOB QUALIFICATIONS AND REQUIREMENTS: Education: Minimal of a Bachelor’s degree in human service or related field.
Experience: Must have experience with working with individuals that have had significantly reduced levels of functioning in primary activities of daily living.
Worker Traits: Must possess a high level of human relation skills and aptitude for effective decision making.
Must possess good planning, organization, and communication skills (oral and written) as well as problem solving skills.
Must be able to handle difficult and stressful situations in a competent manner.
Must be able to maintain confidentiality.
Must possess a valid driver’s license and a willingness to travel as needed to client or organization locations throughout SE MN.

SUPERVISED BY: Community Based Services Director and Crisis Team Coordinator SUPERVISES: None POSITION DESIGNATION: Non-Exempt, Full-Time or Part-Time BENEFIT SUMMARY: Paid Leave Time starts accruing at 5.
08 hours per pay check (pro-rated for employees under Full-Time).
Up to 10 days of paid jury duty leave.
Up to $2,000 Tuition investment and up to $1,500 continuing education funds available.
